PLEASE READ BEFORE COMPLETING THIS FORM:
Form EYE-9 enables citizens to formally renounce their right to blink under the
Ocular Regulation and Eyelid Governance Act 2019, Schedule 18.
Successful completion exempts the applicant from Code 418 monitoring permanently.
Requirements for a valid submission:
(a) The form must be completed in blue ink (this is a digital form, we are aware of the contradiction);
(b) All sections must be completed, including sections that contradict other sections;
(c) The form must be countersigned by a Justice of the Peace, witnessed by a HOOCER-registered Ocular Notary
(there are currently no registered Ocular Notaries — applications closed in 2021);
(d) A £35 non-refundable administrative fee must accompany the form — payment must be made before submission
but payment cannot be made without a valid submission reference, which is issued only after submission;

SECTION 1 — PERSONAL OCULAR DETAILSCODE 418
If your name contains the word "blink" in any language, you must complete supplementary Form EYE-9B before proceeding. EYE-9B does not exist yet.
If you do not have an NBRN, you cannot submit this form. To obtain an NBRN, visit the National Blink Registry portal, which redirects to this form. We are aware of this. It is under review.
LEFT EYE DETAILS:
RIGHT EYE DETAILS (FORM EYE-3 REQUIRED):
Right eye details must be submitted on Form EYE-3 (Right Ocular Registration), which must
be submitted before Form EYE-9, except in cases where Form EYE-9 is submitted first, in which case
EYE-3 must be submitted within 14 days. Both forms must reference each other's submission number.
Neither form has a submission number until submitted. This is being looked into.
Must be between 0 and 200. Must also be accurate. HOOCER reserves the right to dispute this figure. The Dispute Review Panel is backed up until 2028.
SECTION 2 — RENUNCIATION DECLARATION
Note: Option 2 (voluntary only) is not currently recognised under Code 418. Involuntary blinks are also chargeable. Note: Option 3 is also not recognised. Only Option 1 is valid. Note: Option 1 is medically inadvisable. HOOCER is not a medical authority and cannot be held liable for medical advice not given.

SECTION 3 — MANDATORY CONTRADICTORY REQUIREMENTSAll subsections are mandatory
This section was flagged during internal review as "logically inconsistent." The review was acknowledged.
The section was retained on the grounds that removing it would require updating the form reference in
seventeen other documents. The seventeen documents have not been updated since 2021.

The Justice of the Peace must countersign in person at a HOOCER office. HOOCER offices do not accept walk-ins. Appointments are not available. This requirement may be waived if the Justice of the Peace is unavailable, which they always are, but the waiver requires approval from the Justice of the Peace, which creates a logical problem we have documented.
There are currently zero registered HOOCER Ocular Notaries. The register was closed following a review in 2021. It is scheduled to reopen. The scheduled reopening has been rescheduled twice. The current projected reopening is 2028, subject to budget approval. Without a notary, this form cannot be submitted.
